    Uniontown Art club Inc. Membership Application:  Applicant Information: 

    Conditionsl for Probationary Membership Approval

    The applicant agrees to meet the following conditions to gain full membership status: 
    1. Membership:  Membership is only open to artist who are participating in the visual arts and fine crafts as described in our by-laws. 
    1. Membership Requirements: Applicant must have a permanent residence within 50 miles of Uniontown. Must have a phone and email account for contact. Social media access is encouraged. Membership is open to persons 18 years of age and older. 
    1. Probationary Period: Membership is conditional for _6_ months from the approval date. 
    1. Attendance Requirement: Applicant must attend at least _3_ meetings or events during the probationary period. 
    1. Gallery Participation: A member can apply to be juried into the Gallery after the probationary period. 
    1. Code of Conduct: Applicant must adhere to all organization rules and regulations. 
    1. Dues/Fees: Applicant must pay an initial membership fee of $35.00.  This includes a $10 non refundable application fee, and $25 first 1st year membership fee.  Refunded if not approved for full membership. ​
    Acknowledgment and Agreement:
    I understand that my membership is conditional and subject to review based on the conditions outlined above. If I fail to meet these conditions, my membership may not be approved. 
If all of the required fields are filled in, when you click on the submit button the form will disappear.  If you like you can then continue to pay online below or however you choose.

The application fee is $35 which covers your application fee and first year dues.

The dues year is January 1st through December 31st.
